Virtual Group Consultations and Remote Monitoring for LTCs: Call for Practice Participation
North Central London (NCL) Clinical Commissioning Group (CCG) has recently secured funding from NHS England for a programme of work on increasing patient self-management through virtual group consultations and remote BP monitoring.
The programme will target patients with one or more long-term conditions (LTCs) and poorly controlled blood pressure (>150/90), knowing that this group is at higher risk of complications from covid-19.This work will:
- support GP practices to run virtual group consultations to help this patient group learn to better self-manage their LTCs
- provide patients with blood pressure monitors to allow them to remotely monitor their own condition
- increase patient autonomy and capacity for self-management drawing on peer coaching and social prescribing.
The project will create a framework and resources. Practices will be encouraged to adapt processes to suit their local needs; local innovation will be used to support collaborative learning and further development of this model across NCL.
To start this work, we are looking to help practices in NCL develop group video consultation capabilities for patients who have one or more of the following long-term conditions (LTC) - chronic kidney disease; chronic heart disease; or diabetes - and a BP > 150/90.
