The Islington CCG Quality Alert System (QAS)
The QAS is a web-based system created to enable GP practices, clinical and non-clinical staff to alert the Clinical Commissioning Group (CCG) about any provider contractual breaches or other quality issues arising from commissioned services, that have also been been reported directly to the provider. The provider will investigate and respond directly to you in relation to the issue raised.
In addition to provider contractual issues, other quality issues that might be raised could include:
- poor patient experience
- poor quality of clinical care, including medical care, nursing, nutrition, dignity
- transport issues.
The QAS is not designed for making formal complaints to a provider. Please contact the provider directly if you wish to raise a formal complaint.
Islington CCG logs systemic or themed quality issues. The CCG Quality Team will review trends and themes arising, depending on the type of issue it will be raised at the provider CQRG or via a contract monitoring group meeting.
Patient identifiable information should not be included in a quality alert to the CCG.
Serious concerns impacting on patient safety will be taken forward as a matter of urgency with the provider.
In addition, all patient safety incidents in general practice should be reported by the Practice to the National Reporting and Learning System (NRLS) that all providers of NHS funded care use to submit incident information.
