Patients are usually referred to University College London Hospitals (UCLH) or Whittington Health (WH).

If assisted conception is offered, the fertility clinic will refer to tertiary care, usually the Centre for Reproductive and Genetic Health (CRGH) or the Hammersmith Hospital. GPs cannot refer directly to tertiary care.

Eligibility criteria

Inclusions

Specific tests are required prior to referral to fertility clinics and day two to seven oestradiol is also needed.

Please refer to the Infertility Pathway.

Minimum requirements are as follows.

Women

  • Rubella status
  • Haemoglobinopathy screening if indicated
  • Day 2-7 LH/FSH
  • Chlamydia status

How to refer

EMIS form

Referral methods: e-Referral

Prior to Referral

  • Please complete semen analysis and other required testing (see inclusions section above).
  • Please read policy guidance.

Upon Referral

  • Complete the appropriate Infertility referral form in EMIS and send via the e-Referral system.
