Identification and grading of pressure ulcers

Whittington Health has produced an identification and management-made-easy pathway. If a pressure sore has been identified and patient appears at risk of malnutrition please screen using the MUST tool and treat accordingly using the management pathways.

Any pressure ulcer may be an indicator of neglect/abuse; therefore, all should be appropriately assessed to identify any possible safeguarding concerns. If a concern warranting referral is identified, then please refer via the adult safeguarding pathway.

Category (grade) 3 and 4 pressure ulcers are reportable to NHS London as Serious Incidents (SI).

Pressure ulcers can be managed by District Nursing or the Tissue Viability and Lymphoedema service.


Eligibility criteria

Inclusions

Patients

  • with red or broken skin over bony prominence
  • aged 18+

Exclusions

Patients under 18 years of age
